• December 2, 2009: Medical Home and Transition Work Groups held a joint meeting in conjuction with the NEGC Annual Meeting in Portsmouth, NH. The meeting summary is included in the Annual Meeting Report.

• March 4th, 2009: Medical Home Work Group conference call from noon to 1 PM. Click here for the agenda.
  • During the call the group discussed models of tools to enhance communication and co-management of cases between primary physicians and specialists.
    • One example (under development) was provided by Dr. Chris Stille at UMass Memorial Medical Center: "New Patient Referral/Consultation Information".
    • Another example was provided by Dr. Wendy Smith at Maine Medical Partners Pediatric Specialty Care: "Preliminary Genetics Visit Report". Scroll down to see three versions of the form: 1) blank; 2) includes "how to" instructions; and 3) sample of completed form.
• December 4, 2008: Dr. Cooley was the featured speaker at the NEGC Annual Meeting / NERGG Annual Conference jointly-sponsored collaborative session on Medical Home
